Duties and Responsibilities
1)Program Coordinator Role
Responsible for day-to-day operation of Transitional Shared Care Programme (TSCP) for cardiology patients, including the following:
- Screen patients in cardiology clinics for suitability for TSCP based on entry criteria for each condition group.
- Approach clinician in-charge of the patient to review patients suitability.
- Recruit patient into TSCP.
- Liaise with care coordinators in primary care settings (NUP, Primary Care Networks and GPs) for booking of follow-up appointments.
- Communicate treatment targets of the patient with primary care partners.
- Coordinate follow-up appointments at cardiology clinics.
- Provide necessary follow-up with patients who defaulted the appointments.
- Provide support and health counselling to patients.
Track performance of TSCP
- Create database to track all patients, who are enrolled in the programme.
- Record patients attendances at both primary care settings and cardiology clinics.
- Track and document each recruited patients progress and ensure compliance of the clinical protocol.
- Record achievement of treatment targets for all patients.
2)Clinical Audit Coordinator Role
- Collect acute myocardial infarction mortality data and ensure timely submission to MOH.
- Prepare reports and timely submission to the Head of Department and Medical Affairs-Clinical Governance Office.
- Collect the diagnostics variables, procedure medication, intra & post procedure events, comorbidity and lab results as well as treatment plan (optimized medical, revascularization procedure).
- Prepare and submit audited data to the respective authority accurately and timely.
- Perform additional Operations and Administrative tasks as assigned by the supervisor.
Key Requirements
- Bachelors degree from a recognized institution.
- Enrolled Nurse / Registered Nurse / Allied Health Professional / Staff with relevant clinical knowledge will be advantageous.
- 3-5 years working experience in healthcare settings.
- Good verbal and written communication skills.
- Strong interpersonal skills with the ability to engage and work with all levels of staff.
- Have a positive and proactive mindset.
- A good team player who can work independently.
